   Larger heater core is an easy option and requires no firmware rework.

   I had the same issue when I took my printer out of the box. After much of of Mark's Step 3 and hair pulling it turned out to be a faulty heater wire. Yes, one that I checked multiple times before. Its worth taking a very close look.
