Which printer?
That looks like really badly mis-set Z offset for one thing.
If your first layer does not look like this:

You need to work on it a bit...

If it is capable of and doing autoleveling -- you need to make sure the X axis is manually leveled well first or the entire thing will be incorrect. The autoleveling used in the R1 series is not very smart.

 

That looks like inconsistent extrusion to me, but I always refer folks to the visual guides because you have the closest eyes to the problem
