I have included a video. As you can see the fan begins at full speed and then slows and at the one minute mark the fan stops. I can check a minute or so later and fan is running fine only to slow and then stop again.

This is happening with the extruder being stationary and it has caused failed prints over the weekend. Before taking the video I took the fan off and wiped it clean to make sure nothing was blocking the fan. I just changed the fan in the summer of last year. Should I consider the wiring?

It could be the wiring or it could just be a bad fan. They are not very robust.
Check the wiring back to the RAMPS board. If that is good it is probably just a bad fan.
It is a 30mmx30mmx10mm 12vdc fan if you want to try and source one, you can't just go buy parts for the hexagon most places...

    That should work.
