I feel like this piece likely needs support. When enabling support in Cura, it shows the supports being built inside the structure as well. This has previously been fine with voronoi style pieces-but here the holes are so small I don't see how I could practically remove them. Any reccomendations?

Some slicers allow you to manually dictate where the supports go. See if Cura will.

Most of the voronoi designs dont actually require supports. You just need to have bang on temps and bridging/overhangs. Give it a shot without and post pictures, we can try to help you tune in if needed. If you do enable supports on that print you will curse :D. Just have a sharp exacto knife at the ready and clean up any strings (or reprint)
